Hatters Health PCN-Clinical Pharmacist

The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists and technicians in general practice. In this role they will be supported by a senior clinical pharmacist who will develop, manage and mentor them. The post holder will be supported to proactively manage patients within their scope of practice and those with complex polypharmacy with a focus on structured medication reviews in an area of interest. The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services under DES specification. The post holder will support integration between primary care, community and hospital pharmacy to improve patient outcomes. The post holder will be supported to develop their role to become a nonmedical prescriber.

What types of roles are available across the PCN?

Hatters Health PCN offers a wide range of roles including clinical (GPs, Nurses, Nursing associates, Pharmacists and Pharmacy technicians) Health and Wellbeing Roles ( Care coordinators, Social Prescribers) and Non-clinical, (Administrators, Operational and Project leads,  Reception staff). New Roles are added regularly as we expand our services and partnerships.

Do I need previous NHS or primary care experience to apply?

Not always. While some roles require NHS or clinical experience, many of our support, admin, and community-facing roles are open to candidates from a variety of backgrounds. We value transferable skills, a commitment to patient care, and a willingness to learn. If experience is essential, it will be clearly stated in the job description.
